| Description (Service) | The goal of the RCM is to ensure vulnerable populations receive appropriate clinical support and monitoring in the community, including escalation to medical practitioners or acute care, where necessary * helps to reduce unplanned visits to the Emergency Department and/or unplanned re-admissions and develop sustainable self-management techniques for the identified patient populations supported through education and resources * allows patients with COVID, Chronic Obstructive Pulmonary Disease (COPD), Congestive Heart Failure (CHF), asthma, interstitial lung disease (ILD), pneumonia, and other respiratory illnesses, to manage their symptoms at home with technology and nursing support provided to them |
