Description and Service Details
Description (Service) One on one in-home therapeutic recreational activities for adults who live with dementia and are on ADS (adult day services) waitlist or are not currently attending ADS due to barriers

It is not full extend ADS * the program provides maximum of two times per week, one hour each in-home visit, therapeutic recreational and stimulation activities for adults who live with dementia and are on ADS wait list
Hours Administration: Mon-Fri 8am-4pm
Areas Served Mississauga ; Oakville
Boundaries Oakville, South Etobicoke (part in Mississauga Halton) and Mississauga (part in Mississauga Halton)
Eligibility Must meet the following criteria: lives in residential dwelling * has diagnosis of dementia * resides in MH LHIN subregions: Oakville, Mississauga, Etobicoke * needs can be met through this program * eligible for SDL, ADS or ReCharge (but not curretly receiving similar services) * on Adult Day Program waitlist * currently not able to attend ADP due to barriers
