The program aims to improve the health, wellness and quality of life for seniors by improving access to assessment and treatment of incontinence (including urinary incontinence, fecal incontinence, urinary frequency day or night, problems with bowel control, frequent urinary tract infections, difficulty learning how to use or care for a urinary catheter, and constipation)
Continence Home Visit
- patients and/or caregiver(s) will be assessed by a Nurse Continence Advisor who is registered nurse with advanced education and training in continence care
- first visit will last about 1.5 hours * follow up visits, if needed, will be about 30 minutes
- ultrasound test to measure urine volumes and/or physical exam may be needed
- client/Caregiver education is a key part of this program
